How to obtain a police report:
The Records Division is located on the north side of the lobby in the Public Safety Building, located at 225 NW Main Blvd., Lake City, FL. To reach the records clerks by phone, call (386) 758-5432 or (386) 758-5433.
Our customer service window is open from 8:00 am – 5:00 pm, Monday - Friday, and is closed on weekends and City observed holidays.
Copies of reports that are requested through the service window are at no charge. The requestor’s photo ID may be required, dependent on the type of offense involved. Offense reports can also be requested via postal mail and then faxed or mailed for a charge of $2.00. We must receive payment prior to processing the request. Mailing also requires a self-addressed stamped envelope.
Crash reports are NOT public record for the first sixty (60) days following the date of a crash however, they are available immediately to certain parties under Florida State Statute 316.066(3). Eligible parties must provide a valid photo ID and fill out the State required affidavit. There is a five (5) day waiting period before a crash report can be requested from our Records Division, due to quality control processes.

Payment methods include check or money order, made payable to the Lake City Police Department.
Public Records Requests:
Public Records Requests can by requested through the Records Division in person, by mail, phone or by fax at (386-758-5434) or directly through the City Clerk’s Office at (386) 719-5826 or (386)-719-5756. If the Records Division receives a Public Records request, it will be forwarded to the City Clerk’s office for processing. There are charges involved with this type of request. The City Clerk’s office will contact the requestor with estimated charges for the request.
